Welcome to DONMED Family Clinic

DONMED is a primary care and family medicine clinic (POZ) with two locations in gmina Długołęka, near Wrocław. We provide comprehensive healthcare for the whole family — from newborns to seniors — under the national health insurance (NFZ) and through private (commercial) visits.

Most of our members of our staff speak English and are happy to assist you during your visit. For registration, we recommend contacting us by e-mail — someone will always reply to you in English.

What we offer

Family Medicine (POZ)

Our family doctors are fully qualified specialists — not just general practitioners. They independently manage acute and chronic conditions at any age: infections, hypertension, diabetes, obesity, thyroid disorders, heart disease, respiratory conditions, and more. No referral needed for a family doctor visit.

Coordinated Care (Opieka Koordynowana)

A modern NFZ programme for patients with chronic conditions. Your family doctor manages your treatment with the support of a care coordinator, extended diagnostics, dietary and educational consultations, and direct access to other specialists — most available on-site at our clinic.

Dentistry

Our dental practice is located in the same clinic — comprehensive dental care in one place, for the whole family.

Laboratory

Blood and urine tests are available on-site. No need to travel to a separate laboratory facility.

Vaccinations

We offer both mandatory (NFZ-covered) and recommended (private) vaccinations for children and adults, including travel vaccinations.

Driver Medical Examinations

We perform medical examinations required for obtaining or renewing a driving licence in Poland.

NFZ (national insurance) and private visits

Poland's public healthcare system is called NFZ (Narodowy Fundusz Zdrowia — National Health Fund). If you are employed in Poland, self-employed, or registered as unemployed, you are most likely already covered. EU/EEA citizens may use their European Health Insurance Card (EHIC). Patients covered by NFZ pay nothing for standard primary care visits.

If you are not covered by NFZ, or prefer not to sign-in to our facility, we also offer private (commercial) visits with no referral required and flexible appointment times. Please contact us for current pricing.

How to register as a patient

To use NFZ-covered services at DONMED, you need to submit a declaration of choice for a family doctor, a nurse, and — for women — a midwife. This is a one-time process and can be done entirely online.

Option 1 — Online via Patient Account (IKP)

  1. Go to pacjent.gov.pl and log in. You can use a Trusted Profile (Profil Zaufany), e-ID, or online banking (most Polish banks are supported). If you don't have a Trusted Profile yet, you can create one at pz.gov.pl — it requires a Polish PESEL number.
  2. Go to "Moje konto" (My account) after logging in. At the bottom you will see buttons for submitting a new declaration for a doctor, nurse, and midwife.
  3. Click "Nowa deklaracja" (New declaration) next to each role. Verify your personal details and proceed.
  4. Select our clinic:
    • Province (województwo): dolnośląskie
    • Town: Brzezia Łąka or Borowa
    • Select DONMED and any available medical professional.
  5. Sign and submit the declaration using your Trusted Profile or other authentication method. Click "Wyślij" (Send).
  6. Repeat the process separately for the doctor, nurse, and (for women) midwife declarations. The clinic will accept them within a few days.

Option 2 — Paper declarations

You can download, print, fill in, and sign the declaration forms and bring them to our reception desk, or send scanned copies by e-mail.

Important notes

  • Once your declaration is accepted, you can visit any doctor, nurse, or midwife at our clinic — not only the person you named in the form.
  • Changing your declaration is free of charge up to twice per year.
  • Switching from another clinic happens automatically — you do not need to notify your previous clinic.
  • Your first visit should be in person. Please bring any existing medical documentation (test results, discharge letters, prescriptions).
