Nic, co by nas dotyczyło. Deklarację POZ i oświadczenie do badań kierowców wypełniasz w swojej przeglądarce i tam też powstaje gotowy PDF.
My z tych danych nie korzystamy.
A linki do innych stron?

Privacy
In short: we do not collect any information about you. There are no accounts, no advertising, no visitor statistics and no tracking cookies. That is also why you will not see a pop-up asking for cookie consent, since there is nothing to consent to.
What happens to the details I type into a form?
Nothing that involves us. The patient registration declaration and the driver medical statement are filled in inside your own browser, and the finished PDF is created there as well. Your name, PESEL number and address are never sent anywhere, and they disappear once you close the page. We only receive the document when you bring it to the clinic yourself or send it to us by email.
Does the website remember anything?
Only whether you chose the light or the dark theme, so that you do not have to set it again on your next visit. That preference stays in your browser and never reaches us.
Who knows that I am here?
The website runs on servers operated by cyber_Folks. Like almost every hosting company, their servers automatically record basic details about each connection, such as your IP address, the time and your browser type. This is done for security reasons and so that faults can be diagnosed. We do not make any use of that information. You will find more about it at cyberfolks.pl.
The maps on the Contact page come from OpenStreetMap and only load once you click on them. Until then you are simply looking at a picture. After you click, your browser fetches the map itself from the OpenStreetMap servers, which can see your IP address at that moment. The software that displays the maps (Leaflet) is served from our own server, so nothing leaves the site for that.
What about links to other websites?
We sometimes point you elsewhere, for example to online appointment booking, Google Maps or vaccination guidance. Once you follow such a link, the privacy rules of that website apply instead of ours.
